All ID cards available for issue (CAC, Reserve, Retired, Dependent). Appointments are necessary.

Sponsor needs to be present to sign the DD Form 1172-2 or if sponsor has a DOD CAC card and has a CAC enabled personal computer they can log onto https://www.dmdc.osd.mil/self_service to update their information and verify family members for DOD ID card reissuance.   Other options if sponsor is not available: recipient can use a current Power of Attorney, or the DD Form 1172-2 must be signed by the sponsor at an ID card office, or the signature needs to be notarized.

To add a spouse, the following documentation is needed: Marriage certificate, birth certificate, Social Security Card, and one picture ID (driver's license, passport, or federal or state issued picture ID).

To add a child: Marriage certificate, birth certificate, and Social Security Card.

To add a stepchild: Marriage certificate, birth certificate, and Social Security Card.

All documents must be original or certified to be a true copy.
